Complete the Waiver

Visit California is committed to conducting a successful, productive and safe Outlook Forum in San Francisco.
To comply with San Francisco regulations for in-person events, attendees at Visit California’s Outlook Forum conference will be required to wear a mask indoors and show proof of up-to-date COVID-19 vaccination or proof of negative test record.

To attend Outlook Forum, participants must meet one of the following criteria:

MEET UP-TO-DATE VACCINATION REQUIREMENT 
To be considered up-to-date on vaccination for this event in San Francisco, you must:

  1. Have received the second dose of the Pfizer or Moderna vaccine after Sept. 14, 2021, and before Jan. 30, 2022. Per San Francisco's order, these individuals are not eligible for booster yet and, therefore, cleared to attend without a booster. 
    or  
  2. Have received the one-dose Johnson & Johnson vaccine after Dec. 14, 2021, and before Jan. 30, 2022. Per San Francisco's order, these individuals are not eligible for booster yet and, therefore, cleared to attend without a booster.  
    or  
  3. If you were vaccinated prior to the dates above, San Francisco requires a booster. The booster must be received no later than Feb. 6, 2022.

OR PROVIDE PROOF OF A NEGATIVE COVID-19 TEST
San Francisco allows antigen or PCR tests but at-home tests are not allowed. 

  • For antigen tests, a negative test must be taken within one day before your arrival at the event.
  • For PCR tests, a negative test must be taken within two days before your arrival at the event.   

International attendees planning their trips should visit the the U.S. Department of State’s FAQ on vaccination requirements for entry into the U.S.

To be cleared through on-site verification process, participants must bring vaccination record or negative test result and photo ID.

The following are acceptable as proof of vaccination in San Francisco:

  • CDC vaccination card, which includes name of person vaccinated, type of vaccine provided and date last dose administered or similar documentation issued by another foreign governmental jurisdiction.
  • A photo of a vaccination card as a separate document or stored on a phone or electronic device.
  • Documentation of vaccination from a health-care provider.
  • A personal digital COVID-19 vaccine record issued by the State of California or similar documentation issued by another state, local or foreign governmental jurisdiction or by an approved private company.

Outlook Forum Conference Safety

Outlook Forum will implement several additional safety measures to protect against COVID-19:

  • Complying with the government mask mandate for all indoor events.
  • Providing sanitation stations throughout the event space.
  • Providing sufficient event space to allow social distancing.
  • Hosting all conference events at the Hilton San Francisco Union Square, limiting the need for shared ground transportation.
  • Sending electronic alerts to all attendees in the event of a reported infection.

Visit California will work closely with the City of San Francisco and remain in constant communication with attendees to maintain a safe environment and to provide timely updates on any developments that impact Outlook Forum. To expedite your on-site registration process, please read and complete this waiver before arriving in San Francisco.

  • Take stairs or escalators and avoid elevators when possible
